Referrals

 

We accept referrals from all sources including self referrals. It is our goal to make the referral process easy for you and your staff.

Fill out our Referral Intake Form.

Fax or email the forms to us.

Fax or scan the intake form, demographics, progress notes, and any related information to 541-842-2022 or email to frontoffice@retinacarecenter.org

 
 

3.
The appointment is scheduled.

We will contact your patient to schedule an appointment.
The appointment date and time will be forwarded to your office
Progress notes will be faxed to your office following the appointment.
